@brentsimmons Everybody agrees that a situation is bad but practically nobody wants to admit that the situation could have been avoided if we were all more comfortable with even the idea of agreeing to and maintaining rigorous standards. Moving "fast" and accepting "collateral damage" is a much easier, instantly gratifying mindset and approach.
